# Forms & Lead Capture

Add a lead capture form to any Supademo in minutes: build up to 7 fields, gate access by business email or domain, and auto-sync responses to your CRM.

Native forms are lead capture forms built directly into your Supademo as a [Chapter](https://docs.supademo.com/customize/chapters). Viewers fill in the form mid-demo (or before it starts), and every response is emailed to you, saved in your Supademo analytics, and optionally synced to your CRM — no third-party form tool required. ## How to create a form

Creating a customized form takes just a few minutes:

1. Open your Supademo in the editor and add a **Chapter** anywhere in the demo, then choose the **"Form"** option.
2. Add up to **7 form fields**, choosing a type for each: text, dropdown, or number. Drag dropdown options to reorder them.
3. Mark each field as **required or optional**. For phone fields, turn on **"Require valid phone numbers"** so submissions are validated before they go through.
4. Set the form-level access controls: decide whether viewers can skip the form, and whether to limit submissions to business email addresses only (both covered in detail below).
5. Click **Save** to go live with your form.

Viewers see a labelled form layout, and invalid entries show inline errors next to the field, so respondents can correct mistakes without losing their other answers.

## Field types and validation

Forms support three field types, and any field can be marked required or optional independently:

| **Number**   | Numeric answers like team size or seat count     | Accepts numeric input; required or optional                                            |

A form can hold up to 7 fields in total. Keep forms short — ask only for what you'll act on, and use dropdowns where you want clean, comparable answers for segmentation.

## Form access controls

Forms double as an access gate for your demo. These controls determine who gets through:

* **Skip Form** (toggle) — Viewers can bypass the form entirely. Turn this off to make the form a hard gate.
* **Business Emails Only** (toggle) — Restricts demo access to company email addresses, filtering out personal addresses like Gmail.

* **Blacklist/Whitelist Emails** — Control access using custom domain lists. Type domains separated by commas into the textbox provided; the lists scale to hundreds of domains, so you can block competitors or restrict a demo to specific customer accounts.

Submitted email addresses also go through **external email verification**, so invalid addresses are caught before they reach your lead list.

## Where responses go

Every submission lands in three places automatically, with an optional fourth use:

**1. Email notification.** Individual responses are sent to you via email as they come in.

**2. Lead Data analytics.** All responses are saved under the **Lead Data** tab of your Supademo Analytics, alongside each viewer's demo engagement.

**3. CRM sync with AI field mapping.** If you've connected a CRM integration, form responses sync automatically. [HubSpot](https://docs.supademo.com/workspace-settings/integrations/hubspot) and [Salesforce](https://docs.supademo.com/workspace-settings/integrations/salesforce) sync uses AI field mapping: whether you've named a field "Company", "Corporation", or "Business Name", Supademo auto-detects and syncs it to the appropriate CRM field with no manual mapping. Marketo, Zoho, and Pipedrive integrations are also available.

**4. Personalizing demo content with dynamic variables.** Form responses can drive [dynamic variables](/customize/personalize/variables-and-tokens.md), so content within your Supademo — titles, hotspot text, chapters, or HTML content — updates automatically based on what the viewer entered. ### How do I add a lead form to my demo?

Add a Chapter anywhere in your Supademo, choose the **Form** option, add up to 7 fields, and click Save. The form goes live immediately on your published demo — no embed code or third-party form builder needed. Place it after a few value-showing steps if you want higher completion, or on the first step to gate the entire demo.

### Can I require business email addresses?

Yes. Turn on the **Business Emails Only** toggle in the form's access controls to reject personal addresses. For finer control, use the whitelist to allow only specific domains or the blacklist to block specific ones — both accept comma-separated lists scaling to hundreds of domains. Submitted addresses are also checked with external email verification.

### Do form responses sync to HubSpot or Salesforce?

Yes. With a HubSpot or Salesforce integration connected, responses sync automatically, and AI field mapping matches your form fields to the right CRM properties even when the names don't match exactly. Marketo, Zoho, and Pipedrive integrations are also available. Responses are always emailed to you and stored in the Lead Data tab of your analytics as well.
